RISK ASSESSMENT FORM

 It is a condition of the Public Liability Insurance Policy
that a RISK ASSESSMENT FORM must be completed before the start of every match.

Home teams are responsible for providing a form for the referee to complete before the match starts.
 

COMPLETED FORMS TO BE RETAINED BY CLUB SECRETARY

UNTIL INSTRUCTED TO DISPOSE OF THEM.
For this reason, the form available here will be for 2023 season
